a1... by the heart, [the patient is] sharp (= acid) in the mouth. Recognise the above (lit. before) as a symptom of cancer due to gall. He always becomes light (= bitter) in the mouth, ...
a2(likewise he becomes breathless) as if carrying a burden, and he may vomit [and] his nostrils are blocked up. Anxiety (= nausea) besets his body,
a2+[he is] confused and gets (sleepy during the day at inappropriate) moments.
a3He has no appetite for food [and] his body is constantly satiated.
